As Stuart says the infamous "Mills pin" has been fitted in from above because that's where you can get at it, and as a "gotcha" it's tapered which means you can't drive it out from above into the taper. So drilling it out is the way to go from above and then replace with split pin or small nut and close fitting bolt on the hole size. Make sure there is no slop between pin and hole.
