metal swarf in gearbox bell housing

So, the gearbox is out and looks in pretty good nick....except for the small pile of metal swarf in the base of the bell housing. eventually I found the source, a half moon shaped damage on the clutch throw out sleeve but I can't see what has caused it. Anyone come across this before and the likely cause?
